THE EMOTIONAL ARC DESIGN FRAMEWORK (EADF™)
Transforming Clinical Learning into Human Experience
Healthcare simulation has traditionally focused on clinical accuracy, procedural competence, and technological fidelity. While these elements remain essential, many technically excellent simulations fail to create lasting impact because they do not intentionally address the emotional dimension of learning.
The Emotional Arc Design Framework (EADF™) is a structured methodology for designing simulation experiences that integrate clinical challenge, emotional engagement, human connection, and reflective learning. By intentionally designing moments of tension, uncertainty, empathy, decision-making, and resolution, simulation educators can create experiences that learners remember long after the scenario ends.
The framework helps educators move beyond asking:
“What should learners do?”
and instead ask:
“What should learners feel, think, do, and remember?”
THE EADF™ MODEL
The Emotional Arc Design Framework consists of three core phases that shape the learner experience.

1. Tension Rise™
The scenario introduces uncertainty, incomplete information, competing priorities, or emotional triggers that capture attention and create engagement.
The goal is not to create anxiety for its own sake, but to establish meaningful cognitive and emotional investment in the patient and situation.
Key Design Elements:
- Clinical uncertainty
- Time pressure
- Human connection
- Emerging concern
- Emotional engagement
2. Breaking Point™
The learner encounters a critical moment requiring judgment, prioritization, leadership, communication, or ethical decision-making.
At this point, clinical reasoning and emotional response intersect.
The Breaking Point™ often includes an Emotional Anchor™—a statement or event that transforms the patient from a diagnosis into a human being.
Examples:
“Doctor, am I going to die like Maher?”
“Please don’t let my daughter see me like this.”
The Breaking Point™ represents the emotional peak of the learning experience.
3. Resolution & Release™
Following intervention and decision-making, the scenario moves toward clinical stabilization, emotional release, and reflective processing.
This phase provides closure while preparing learners for debriefing and meaning-making.

THE FIVE CORE DESIGN TECHNIQUES

1. Emotional Anchor™
A memorable statement, concern, fear, or hope expressed by the patient that creates emotional connection and meaning.
Purpose: Transforms the patient from a clinical problem into a human story.
2. Narrative Tension Point™
A deliberately designed moment where uncertainty, urgency, or emotional pressure begins to increase.
Purpose: Creates engagement and prepares learners for critical decision-making.
3. Silence Moment™
A structured pause following an emotionally significant event.
Purpose: Allows learners to experience uncertainty, empathy, responsibility, and reflection before acting.
4. Emotional Arc Blueprint™
A visual mapping of learner emotions throughout the scenario, including entry, escalation, breaking point, resolution, and reflection.
Purpose: Ensures emotional progression is intentionally designed rather than occurring by chance.
5. Feeling Check Debrief™
A structured debriefing strategy that explores the emotional dimensions of performance.
Example Questions:
- What were you feeling at that moment?
- How did those emotions influence your decisions?
- What would you do differently next time?
Purpose: Connects emotional experience to clinical learning and future practice.
